  • Select the +MORE button

  • Select view consumer notes to review all previously documented notes

  • Select new consumer note to add a note

  • Select consumer from drop down box

  • Select note type from the following list:

    • Behavior Notes - When completing a behavior service, details of the target behavior will be documented under behavior notes.

    • Contact Notes - Details of phone, face to face, email or mail contact to or from a guardian or case manager.

    • Incident Follow up Notes - Details of incident follow up that were not documented on initial incident report.

    • Medical Entry Notes - All medical related details.

    • Medical Visit Notes - All medical appointment details.
