Medications will be disposed of when discontinued, expired or if the consumer has a change in dose and the current pharmacy does not complete this:

• 2 staff will be present during all medication destructions

• Log into Rtask

• Select Clinical tab - dispose medications

• Select consumer from drop down box

• Select dispose single medication or dispose multiple medications (only completing for one consumer at a time)

• Select medication from drop down box or from created list if multiple

• Fill in RX number from prescription (or indicate ‘PRN-no RX available’)

• Fill in corresponding pill count being disposed of (or indicate amount of liquid, cream or ointment)

• Select reason and disposal method from drop down boxes

• Select witnessing staff from drop down box

• Witness will then type in their Rtask password

• Save
